entry-gate: drop the now-false rank-concentration boot warning (plan 005 step 4)

Rank concentration now runs on BOTH paths via the shared gate, so the warning
that it was 'enforced ONLY on the dead fast-trigger path' is false. Removed it
and folded rank top-N state into the [boot] entry-gates summary line.
